1. Prescreening is performed by the Audibel Hearing Center, who donates their time to work with patients. Call Audibel Hearing Center and set up appointment for screening. (217)373-1500

2. “First Call for Help” can also help you with screening for financial qualification; at 217-352-0099.
Be sure to tell them “Champaign Lions Club” is sponsoring your screening and that you will need our help if you qualify.

3. Audibel will help you fill out the Starkey Hearing Foundation Form or you may fill it out prior to visiting Audibel. You can also personally see if you qualify financially by going to the Starkey Hearing Foundation website to view the financial qualification form:  http://www.sotheworldmayhear.org/hearnow/
First Call for Help can also help you fill out the forms to apply for financial assistance prior to making your appointment with Audibel.

4. Audibel will then inform the Lions Club if you qualify and we will send them the check for assistance up to $250
